If you’ve been injured in a motor vehicle accident, you can lodge a NSW Green Slip claim with us if you’re either the at-fault driver and insured with us, or injured and the vehicle most at fault is insured with us.

If you submit a claim within 28 days of the motor vehicle accident, you may be entitled to receive some of your benefits immediately from the date of the accident. If you submit later than this, some of your entitlements may only begin from the date the claim is made. At the latest, you should lodge a claim within three months of the date of the accident.

If the vehicle most at fault isn’t insured with us for CTP, you can contact the State Insurance Regulatory Authority (SIRA) to determine who they’re insured with.

If you don’t know whether we’re the CTP insurer of the vehicle most at-fault, give us a call and we’ll be happy to help.
If you believe you’re eligible, it’s easy to lodge a NSW CTP Green Slip claim with us.
You’ll need to get hold of the registration details of the vehicles that were involved in the accident.
Report the accident to the NSW Police by calling 131 444, unless they attended the accident scene.
Download, print, and fill out the relevant claim form. Once completed, sign and send to us.
  • You’ll need to include a medical certificate from your GP detailing your fitness for work with your claim form. A copy of the certificate of fitness is available from SIRA (PDF, 639KB)
  • The claim form should be sent to us as soon as possible, as the law sets time limits for making a claim. You should provide the form to us within three months of the date of accident, or within 28 days to maximise your entitlement to benefits.
  • If you submit a claim within 28 days of the motor vehicle accident, you may be entitled to receive some of your benefits starting from the date of the accident. If you submit later than this, your treatment and care benefits may still begin from the date of the accident, but benefits for income support may not begin until the date the claim is made.

If you’d like to make a claim for common law damages, complete this form and send to us.
If you wish to make a claim for funeral costs relating to someone who has died after a motor accident in NSW, complete and send this form to us.
Complete this form and send to us to request compensation for the death of a dependant from a motor accident in NSW.

You can send your claim by email, but remember to also send the original copy in the mail. You can also email us if you have an existing claim, but make sure to include your claim number and name as the subject line. Send a fax to 02 9266 7116.

Once you’ve filled out the relevant form and sent it to us, we’ll do the following:
  • Contact you within three business days after we’ve received your claim form
  • Send you a letter advising you of your claim reference number and the name and contact details of the person who will be responsible for supporting your recovery journey
  • Keep you up to date on the progress of your claim and encourage you to contact us if you have any questions
  • Make enquiries into the circumstances of the accident and determine who’s at fault
  • Tell you within four weeks of receiving your claim whether the claim has been accepted or not
  • Let you know of your entitlements and your claim will be assessed against those entitlements

Yes, the legislation imposes time limits for making a claim. For a personal injury statutory benefits claim, you must submit a claim no later than three months from the date of the accident. If the claim’s made outside of the three months, we may need more information to consider accepting your claim.

We’ll contact you within three business days after we’ve received your claim form. We’ll send you a letter advising you of your claim reference number and the name and contact details of the person who will be responsible for supporting your recovery journey. We’ll also keep you up to date on the progress of your claim and encourage you to contact us if you have any questions.
We may need to contact you to obtain an update on the progress of any treatment and we may also need to obtain more documents from you to assess your claim. We’ll discuss with you the need for any of this additional information.

Make sure the accident has been reported to the police (if they didn’t attend the accident.) You can report the accident to the NSW Police by contacting the Police Assistance Line on 131 444, 24 hours a day, 7 days a week. Tell the police if you were injured in the accident and get the Police Event Number for the accident.

Get hold of the registration details of the vehicles involved in the accident. Once you have this information, you can complete the Personal Injury Claim Form (PDF, 326KB). If you have any questions about getting this information, call us on 1300 131 319.

It may still be possible to make a claim if you were injured in a motor vehicle incident and you didn't get registration details of the most at-fault vehicle, or that vehicle wasn't insured. To find out more, call CTP Assist on 1300 656 919.

If you wish to make a personal injury claim, print, complete and send to us a Personal Injury Claim Form (PDF, 326KB). If you’re unable to print this form, contact us and we’ll send the form to you. The form should be sent to us as soon possible as the law sets time limits for making a claim. Get in touch if you need help or have any questions.

The form includes a medical certificate, which is needed to be completed and signed by a registered medical practitioner.

If you’re making a claim for funeral expenses, complete an Application for Funeral Expenses (PDF, 331KB). If you’re claiming for losses or expenses incurred as a dependant of a person who died in a motor vehicle accident, complete a Compensation to Relatives Claim Form (PDF, 310KB).

Once the claim form is completed, you’ll need to sign it. If you’re under 18 years of age, your parent or legal guardian over the age of 18 should sign it on your behalf.

We’ll make enquiries into the circumstances of the accident and determine whether the vehicle most at fault is insured by Allianz. We’re required to determine liability as soon as possible. We’ll let you know your entitlements and your claim will be assessed accordingly.

If you’ve been injured in a motor vehicle accident that occurred before 1 December 2017, you may be able to make a CTP claim if your injury is a result of a motor vehicle accident that was not wholly your fault. If you’re wholly at fault, your CTP insurance may provide limited cover – up to $5000 in treatment costs and lost income incurred in the first six months after the accident.

In both cases you need to claim against the insurer of the vehicle most at fault in the accident. If we’re that CTP insurer, contact us on 1300 131 319.
